Join Our Data-Driven Partnership in homelessness!

The problem: Currently, care systems are disconnected, and there is limited sharing of information and collaboration between doctors and professionals. So many times care is provided thinking that one size fits all. The results are poor patient outcomes and waste of money.

The solution: The development and release of integrated management dashboards have provided a valuable resource for service providers, commissioners, and policy officials involved in the delivery of out of hospital care for people who are homeless (link here). These dashboards offer a user-friendly way to access and utilise data, fostering better decision-making and accountability. More in our flyer here.

For an example of impact, by utilising the local integrated management dashboards, service providers in Oxford effectively showcased its ability to not only replicate but also enhance the outcomes demonstrated in the initial effectiveness studies. Notably, there was a remarkable 56% reduction in A&E visits during the year following a stay in a step-down house, in contrast to the 18% figure reported in the original study. These results are coupled with favourable economic outcomes, including decreased emergency care utilization and improved care planning, resulting in the release of resources for the NHS and other public budgets. The data helped to secure additional funding to secure the sustainability of services on a larger scale for two years.
